Guided Pathway for requesting a motion to change on consent – Form 15C or 15D
CLEO’s Family Law Guided Pathways help you fill out the court forms you need for your family law matter.
You can use this pathway if you want to request a motion to change a final order or support agreement, and you and the other party agree to this change.
The pathway asks you questions, gives you information to help answer them, and puts your answers into the required forms. The pathway is free to use.
When you’re finished, you can save or print your forms. You will also get instructions about how to give them to the court.
Who is this guided pathway for?
This pathway is for individuals who want to change the terms of the final court order or support agreement, and the other party agrees to the changes.
If you have a Final Court Order, you can ask to make changes to
- decision-making responsibility, which used to be called custody
- parenting time, which used to be called access
- child support
- spousal support
- other family law orders
If you have filed your support agreement with the court, you can ask to make changes to:
- child support
- spousal support
If you have a Notice of Recalculation from the online child support service, you can ask to make changes to child support.
Who is this guided pathway not for?
If the other party does not agree with the changes: You should not use this pathway if you and the other party do not agree to change or end a final order or support agreement. There is another pathway for this.
If you have not filed your agreement about support: You should not use this pathway if you want to change child support or spousal support and you have not filed your support agreement with the court. There is another pathway for this.
If you only have a temporary order (not a final order): You should not use this pathway if you want to change a temporary court order.
If you have a written agreement about parenting: You should not use this pathway to ask the court for ‘changes’ to parenting if there is no Final Court Order about parenting. Instead, you should start an Application (Form 8) to ask for a court order about parenting. There is another pathway for this.

Using the pathway
It can take less than 1 hour or 4 hours or more to complete the pathway depending on your situation and what you’re asking the court for.
It can take less time if you have all your documents ready when you start. The pathway is divided into steps. At the beginning of each step, there’s a list of the information that you need to continue.
You can sign up to create an account or use the pathways as a guest. If you choose not to create an account, the system will time out after 4 hours. You must create an account if you want to save your work on the system or use the other pathways.
